Family Health Plus (FHPlus) is a health insurance program for uninsured adults in New York who are 19 years old or older. To be eligible for FHPlus in New York you must:
• Be a New York State resident
• Not be eligible for or enrolled in Medicaid or Medicare
• Have a gross household income that is at or below the program’s eligibility limits
• Not be receiving health coverage from another source (including health insurance you may have outside of New York State)
• Be 19 to 64 years of age
Your gross household income must be at or below the following income limits to be eligible:
• Household of 1 – $32,318/year
• Household of 2 – $43,260/year
• Household of 3 – $54,202/year
• Household of 4 – $65,144/year
• Household of 5 – $76,086/year
• Household of 6 – $87,028/year
• Household of 7 – $97,970/year
• Household of 8 – $108, 912/year
For households with more than 8 people, add $11,000 (before taxes) for each additional person.
