Who is Eligible for Family Health Plus in NY?

I am trying to find out if my family is eligible for family health plus in New York. We are a family of four and our income is about $45,000 a year. Can anyone tell me if we would be eligible?

There are several requirements that must be met in order to be eligible for Family Health Plus in New York. First, the applicant must be a resident of the state. Second, the applicant must be a citizen of the United States or a lawful permanent resident. Third, the applicant must be between the ages of 19 and 64. Family Health Plus (FHPlus) is a health insurance program for uninsured adults in New York who are 19 years old or older. To be eligible for FHPlus in New York you must:

• Household of 3 – $54,202/year

• Household of 7 – $97,970/year
• Household of 8 – $108, 912/year

For households with more than 8 people, add $11,000 (before taxes) for each additional person.
