You may be required to get an SR-22 if you are convicted of certain traffic violations, including:
• DUI/DWI
• Reckless driving
• Refusal to submit to a breath test
• Driving without insurance
If you are required to get an SR-22, you must notify your insurance company and they will file the form on your behalf. The SR-22 must be kept on file with the DMV for a period of three years.
